A Post Consult Plan.
Our communication and advice doesn’t conclude at the end of a consultation. Our team offers patients detailed post consult plans for success. Patients are provided with consult notes, follow up plans, and a course of action to overcome their breast feeding obstacles. Every time.
Refer a Patient
We accept referrals from physicians and midwives for OHIP-covered breastfeeding and infant-feeding assessment and support.
For the fastest and most secure referral process, submit your referral through our secure Ocean eReferral form. Please enter the Mother/Birthing Parent as the patient and include Baby’s information within the referral form.
If there is a time-sensitive clinical concern, please include this clearly in the Reason for Referral section.

We also accept referrals by secure fax at (866) 483-1705. Please include Mother/Birthing Parent and Baby’s identifying information, your contact/billing information, and the reason for referral.
